What Causes Pimples on the Labia and How to Treat Them?

Acne on the labia majora, or vulva, can be uncomfortable and embarrassing, but it’s usually not serious. Like the skin on other parts of your body, acne can develop on the vulva and labia majora for a variety of reasons, including clogged pores, sweat, and hormonal changes. To ensure proper care and comfort, it’s important to understand what causes acne, how to treat it, and when to see a doctor.

To know the causes of labia majora breakouts and tips to get rid of them, OnlyMyHealth team  spoke to Dr Neha Khandelwal, Head of Obstetrics and Gynaecology, Cloud Nine Group Hospital, Kailash Colony, New Delhi.

Clogged hair follicles 

One of the most common causes of acne on the labia majora is clogged pores. “The vulva, like other areas of the skin, contains hair follicles that can become clogged with sweat, oil, and dead skin cells. When this happens, breakouts can form that appear as small lumps or whiteheads,” explains Dr Khandelwal.

Dr. Khandelwal says that tight clothing, especially synthetic underwear and non-breathable fabrics, can cause friction and irritation around the labia, which can lead to tiny, pimple-like bumps, especially in areas of skin that are frequently rubbed.

Sweat and bacteria 

The genital area is especially prone to sweating in hot weather, while exercising, or when wearing tight clothing. “When sweat mixes with bacteria and gets trapped in the pores, it can cause acne. This condition is commonly called folliculitis, which is inflammation of the hair follicles due to infection,” says Dr. Khandelwal. 

Hormonal changes 

Hormonal changes during menstruation, pregnancy, and even menopause can cause acne on the labia majora. “Just like the face, the genital area can respond to hormonal changes by producing more oil, which can lead to clogged pores and acne,” Dr Khandelwal points out.

Bury the hair 

After shaving or waxing your pubic area, you may experience ingrown hairs. Ingrown hairs occur when hairs grow back into the skin instead of growing out of it. This results in acne-like bumps that can be red, swollen, and painful.

Skin conditions 

“Certain skin conditions, such as hidradenitis suppurativa, a chronic inflammatory skin condition, can cause acne-like lesions in areas such as the groin or labia majora,” explains Dr. Khandelwal. This condition differs from regular acne in that it requires medical attention and causes deeper, painful swelling.

Infect 

Though less common, infections like bacterial vaginosis or sexually transmitted infections (STIs) can cause acne-like bumps in the genital area. “If the lump is accompanied by other symptoms like abnormal discharge, pain, or itching, it’s essential to see a doctor,” she warns.

Treatment for acne on the labia

Here are some tips from Dr Khandelwal to get rid of painful acne on the labia majora. 

Keep the area clean 

Gently washing the genital area with warm water and a mild, fragrance-free soap can help reduce sweat and oil buildup. Avoid rubbing vigorously, as the skin on the labia is very delicate and can become irritated.

Wear loose, breathable clothing 

Wearing loose-fitting cotton underwear and avoiding tight, synthetic undergarments can help prevent acne caused by friction and sweat. “Cotton allows the skin to breathe, reducing the risk of irritation,” says Dr. Khandelwal.

Avoid irritation when shaving 

If you shave or wax your pubic area, consider using good shaving techniques to reduce the risk of ingrown hairs and irritation. “Use a clean, sharp razor and shave in the direction of the hair growth, then apply a gentle, fragrance-free lotion. If you’re experiencing recurring ingrown hairs, you may want to consider switching to trimming instead of shaving,” advises Dr Khandelwal.

Use a warm compress 

Applying a warm compress to the affected area for 10 to 15 minutes several times a day can help soothe the skin and allow the acne to heal and drain naturally. This can also help reduce swelling and discomfort.

Don’t pop or pick pimples 

No matter how tempting it may be, avoid picking or popping pimples on your labia majora, as this can introduce bacteria into the area, causing infection and scarring.

Medicines that can be purchased without a prescription 

You can also use over-the-counter acne medications that contain ingredients like benzoyl peroxide or salicylic acid, but be careful when using these products near sensitive genitals. “Always talk to your doctor before using any such products to make sure they’re safe for use in that area,” stresses Dr. Khandelwal.

Stay hydrated and eat a healthy diet 

Drinking enough water and eating a balanced diet rich in fruits and vegetables can help improve your skin’s health, while avoiding too much sugar and processed foods can also reduce your risk of acne.

When to see a doctor

Most acne on the labia majora is harmless and can be treated with home care, but in some cases, medical attention is required.

  • If your acne is unusually large, painful, or doesn’t improve with home treatments.
  • If you have signs of infection such as pus, fever, or increased redness or warmth.
  • If you suspect that the swelling may be related to a sexually transmitted or other infection.
  • If your acne keeps coming back, it could be a sign of an underlying medical condition.
